If I understand clearly, you are talking about the fourth hand, with an arrow ? This one is for GMT, or Universal Time Coordinated, it indicates the same time as the hour hand, but on a 24 hours scale. If you travel, you just have to turn the bezel (with the 24h indication) to get the local time while keeping your watch set on your home time.
